Approval of 'Title page package' and definite defence date - Amsterdam UMC/UvA

For whom:

AMC/UvA PhD candidates who have completed steps 1-5

Aim:

Confirmation from the dean that you have met the requirements for admission to the PhD defence ceremony and have permission to proceed with publication of the doctoral thesis

When:

No later than 9 weeks before the provisional defence ceremony date

Timeframe:

One to two weeks for the dean to sign off on the admission to the PhD defence ceremony and for the Office of the Beadle to confirm the reserved doctorate conferral date

To do:

The ‘Title page package’ is delivered by or on behalf of the supervisor in one batch of documents by post or in person to the Office of Doctorate Affairs

Necessary documents:

The academic title page generated by the Office of the Beadle (see under Preliminary defence date)
Printed copies of all doctoral thesis assessment forms completed by voting committee members (Appendix A, in English or Dutch)
UvA Faculty of Medicine ‘Specific assessment criteria’ form (only available in Dutch). This form asks for information about the number of published papers in the thesis, number of co-authorships, and the plagiarism screening. It must be completed and signed (electronically or hard copy) by one supervisor
The plagiarism screening results report emailed by phdscan@amc.nl to the supervisor, and if similarities were found also the similarity report evaluated and signed off by the dean’s delegate
