In recent years, increasing attention is given to a patient’s responsibility for, and control over their own health and healthcare. Value-based healthcare aims to provide care in such a way that it produces the most added value for the patient. As such, it takes the patient’s opinions and experiences into account, while organizing the multidisciplinary care efficiently around the patient.
Digital care pathway
One way to properly involve and inform patients during their care process is to use a customized and personalized digital care pathway (DCP). This DCP enables to give accurate information at the right time and place to both patients and care professionals. In addition, it can provide healthcare professionals and patients with the information they need to support the patients’ self-management. This requires a good overview of where the patient is in the care process and insight into the situation of the patient, as well as the explicit wishes and needs of the patient.
The aim of this research is to gain knowledge about the development and added value of customized DCPs for end-users and the care process itself. The design takes places in co-creation with the end-users using adaptive design as the primary method. During the adaptive cycles, qualitative and quantitative evaluations are conducted to learn about the development process as well as its effectiveness. One of the products of this project will be a comprehensive guideline for the development and implementation process of personalized care pathways.
