To do:
If you can present a DUO uittreksel diploma or an exemption from the legal educational requirement (step 1), you can apply for admission to the doctoral programme by sending an email with required documents to promotiezaken@amc.uva.nl.
Necessary documents:
- Completed and signed 'Request for admission to the doctoral programme form' - English/ Dutch. See 'Tips' for instructions completing the form correctly
- A 'DUO uittreksel' of your Master's degree or the Doctorate Board's decision exemption legal educational requirement (step 1)
- A valid original ID, such as a passport (stating no more than full name, date and place of birth and validity; a driving licence is not accepted)
- Please indicate that you are registered with the Amsterdam UMC Doctoral School in your email to the Office of Doctorate Affairs (see requirements here below under 'Important')

Instructions for completing the ‘Request for admission to the doctoral programme’ form:
- If your supervisor is not a full professor, they must obtain the right to propose a person for a doctorate (ius promovendi), provided they meet the criteria. For further information contact the Office of Doctorate Affairs
- If one of your supervisors is affiliated with a foreign academic institution, they should hold a position comparable to that of a full or associate professor qualified to propose a person for a doctorate (ius promovendi) - a document confirming this statement is required
If your co-supervisor is not a full, associate (UHD) or assistant professor (UD) with a doctorate, but is an expert with doctorate, they must provide a justification for the appointment (on the back of the form). This justification is deemed sufficient if the supervisor confirms that:
- The co-supervisor holds a doctorate/PhD (specifying the year, university and – if possible thesis – title), and
- The co-supervisor is an expert in the field or a subarea of the thesis, and
- The co-supervisor fulfils a genuinely supervisory role; this requires a detailed description of that role, and
- Indicates where or for which organization the co-supervisor works (affiliation)
- If your co-supervisor is affiliated with a foreign academic institution, also enclose a document confirming their appointment as a full, assistant or associate professor and/or MD holding a doctorate
Important: At the moment of requesting for admission to the doctoral programme:
- You need to be registered with the Doctoral School
- The appointment of the supervisor(s)/co-supervisor(s) is fixed
- At least 16 weeks prior to the defence, it is certain that you will meet all the requirements for the PhD thesis
