The ACTIVE trial is a multicenter randomized controlled study evaluating a personalized multidisciplinary eHealth intervention for patients undergoing total or unicompartmental knee arthroplasty. The project was developed in response to large variability in postoperative activity recommendations and inconsistent recovery patterns across Dutch hospitals.

The intervention is a eHealth module based on a previously developed Delphi consensus algorithm and provides tailored timelines for resuming 27 daily life activities, including work and sports. Through the ikHerstel app, patients receive personalized guidance aligned with their recovery speed, aiming to optimize safe reintegration into daily functioning.

The trial assesses effectiveness and cost-effectiveness compared to usual care, focusing on return to activities of daily living, work participation and sports. A comprehensive process evaluation examines implementation fidelity, patient adherence, and user experiences. Results contribute to evidence-based postoperative guidance, aiming to standardize recovery recommendations and enhance long-term physical activity levels after knee arthroplasty.
